I don't have one, but they are pretty easy to make.
Just buy some plywood and glue a breadboard on there together with a small metal/plastic/acrylic panel with potentiometers and switches.
Its not really harder than making a pedal.

Casper electronics makes something very similar but with 1/8" jacks
It includes power , an output power amp , pots and a "vactrol "

I think it's called the OHM synth

As per Havaden's response.
I just have a bit of wood, some breadboard glued on it, and a couple of rows of terminal strip.
It's not the best, maybe I could streamline it by attaching some sheet metal that I can screw the pots, switches and jacks to, but it does the job for now.
