PCB for latching relay driver?

I'm working on a pedal with five independent DPDT relays in it, and I of course need a circuit to drive each of them.  I figure R.G. Keen's 4069 latching relay driver makes the most sense.  I've seen schematics and even board layouts based on this circuit, but does anybody sell a PCB for it?
