Stereo Transformer Isolator

I have some Jensen JT-11P-1 transformers leftover from a studio project. I was hoping to use them for transformer isolation between my two amps. I already have a stereo signal split by stereo pedals. I want to make this as basic as possible. The reason being is that right now I use both amps always on (don't need a switcher). If I see the need I can just make kill switches to mute a certain amp. Eventually I may include switching into a midi controlled relay switcher box. So, with that said, I'd like to just make a little transformer isolation box that will sit underneath my board that I can connect whatever switcher I end up with. Kind of a modular approach.

1) Should I just wire it up passively like schematic posted by Jensen (minus the Out 1)? I want to avoid high end and volume loss. I use 16ft cables from the board to my amps and do have a couple buffered bypass pedals in my chain.
2) Am I better off with a transformer on each signal or do I only need one?
3) If a buffer/amplifier is needed, can I use a standard unity gain buffer or do I need to make up for loss through the transformer? I have a couple populated pcbs hanging around with the Cornish buffer on them and don't mind using them if needed, but I believe they are unity gain. I really like the sound of these and wouldn't mind them last in line before the amp. But I would like to know if they are necessary and/or if they'd need to be modified for this.

I am not an expert, but here are my thoughts:

1) I would just run it passively per the application note. If you already have a buffer in your chain, it should be fine.
2) if you are just looking to break ground loops between two amps - you only need one.
3) I don't think it is necessary to add another buffer or gain stage
