Appears to have 2 ground pads, take your pick or use both, top left corner and top right corner.
Input appears to be 2nd pad from top left.
Output appears to be 2nd pad from top right.
Anyone else see it that way?

If you use a DPDT switch, questions 1 and 2 have the same answer: http://www.geofex.com/article_folders/millenium/millen.htm. Or you can use a 3PDT, so you don't need to use the millenium bypass.

For question 3, you can use this wiring diagram.
