Small Stone Issue H/J Mod for True Bypass, LED

After doing a huge number of searches online, I haven't managed to come across a diagram of how to mod an old Small Stone for TB that is still active/available. I tried doing it this morning just based on instinct and sussing out a couple of things, but all I'm getting through is feedback/resonance when I turn the pedal on, without any of the rest of the wet signal. Additionally, I have a small amount of leak when I switch the pedal on without power plugged in, with a little bit of normal signal still coming through to the amp.

I followed a standard wiring diagram (in this case, Madbean's with also grounding the output http://www.madbeanpedals.com/tutorials/downloads/StandardWiring_MBP.pdf), and I verified that the pedal worked with the new power adaptor before switching out the footswitch. I suspect the problem has to do with where I'm pulling board output. Since the old footswitch either directed the signal directly to the output jack or into the board, where it then went from the bottom left lug of the color switch to the output jack, I used the bottom left lug of the color switch as the "Board Out" when wiring everything.

I figure this is probably wrong, but what should I do to fix it? Also, what should I do with the other wire than comes off of the input straight into the board (on my board a white wire - pictures below)? Any help would be great.
