Boss 5 lug DC input Jack

Hi Everyone.  I just got a used pedal that was supposed to be a DS-1, that I was going to Mod. 

I opened it up and it has a an FZ-3 board in it, but the dc input jack is broken on it.  I have some of the 3 lugs from other projects that would fit it, but this board has a 5 lug input jack.   Its a much wider jack than the 3 lug jacks.  It looks like they tried to carve the plastic away to get it to fit into the DS-1 body.

Do any of you know where I can get one of the 5 lug jack(I haven't been able to find on using google, or looking at the sites I buy pedal parts from), or do you know I could make a 3 lug jack work in its place.
