Neutron and 3PDT Switch

I'm having a bit of trouble figuring how to wire a 3PDT switch with the Neutron.  I'm using the Blue Magic Layout as a guide with the LED connection to the pole on the lower left, 1-3 (using matrix notation).   Pole 1-1 and  2-1 are connected.  3-1 is connected to 2-3 which is connected to R4 on the board.  1-2 is connected to output ground.  3-2 is connected to C14 and R18 then to output tip.

My question is, where do I connect the mode pole (center pole 2-2?) and what is the other pole for.  Hope this isn't too confusing.
