Does anyone have the layout for the phaseshifter foil template from GGG?

 [Im trying to debug the EPFM circuit from GGG. I have built the circuit,but it doesnt work. Does anyone have the EPFM phaseshifter component layout that goes with the foil template that is currently available on GGG(Generalguitargadgets)site. I really want to complete this circuit and get it working. Also,if possible,does anyone know the orientation of the ICs on this phaseshifter layout? Im using a 4136 for the top IC and 2 dual opamps for the other 2. THANKS

I think this is what you are asking for... from the Craig Anderton book, right? I recommend the book, its pretty sweet.
