What offboard wiring to use for tonepad tubescreamer? ts808/ts9 switch?

Hi all,
I'm starting my first pedal (tonepad tubescreamer).  I'm confused on what offboard wiring to use.  I printed out the wiring diagrams from tonepad but i'm not sure which one to use.  i'm using a status LED so that narrows it down.  I think i should be using the #5 diagram as it matches the switch that i ordered from small bear.  Is that correct?

Also, i want to use a single switch to change between the ts808 and t9, and also a single switch to change the clipping.  I was sure i had stumbled across a wiring diagram for how to wire both of these mods.  can anyone point me to those links or any diagram it our for me and tell me what switch to use (3pdt on-on)?
