from ground take voltage readings from each transistors emiiter base collector [what have you] and post them.
  Swap the opamp.
 Test all the 'regular '[?] stuff...
 Is strange behaviour, sounds like you remember the conditions well of the circumstances around the time of the failure...seems uncommon for 'that' to have 'this' result...DC blocking caps should keep one power supply on one box from frying the PS on the other box....
 Box...I've had lotsa troubles with boxxing...I've been beaten by boxes before.
 Sometimes retro is the only way for me to reduce the variables so I start takeing this off and then that off...still testing and stuff but eliminating down to the 'bare' ckt...sometimes just using test clips like audio probe 'n such...ya get yer voltages 'right' on the actives and signal to the input...it's 'gotta' work.

I measured the voltages of My Muff Fuzz and here they are:

Q2

C= 3.5
B= 1.8
E= (i forgot, somewhere 1.*)

Q1

C= 1.6
B= 0.6
E= 0
