Newbie alert!

In the schematic http://www.premierguitar.com/Stream/StreamImage.aspx?Image_ID=151F83BD-D43D-456C-BB03-2A1182CC6C30&Image_Type=image  there is a 1m resistor (R5) coming from 3 of the op-amp and going to 4.5v, and my question is where is the other 4.5v to be connected?  Not sure if part of the schematic is not shown or should just assume the connection is to?

Thanks   ???

Schematic that you posted is missing a biasing network. And by network I mean two resistors and a capasitor. Its doing the crucial job to give you that 4.5v. Look for other schematics that show it.

What you need to add is R2, R3, and C1.   R4 would be the 1M you are referring to on your schematic.

This is the bias network...sometimes it is assumed, altho it's courteous for whoever is drawing a circuit to show it for you!  ;)
