Wiring for 3PDT daughterboard

Hi,

I am having a hell of a time with this. I have built the AD-3208 using the GGG layout (http://www.generalguitargadgets.com/projects/20-modulationecho/48-ad-3208). It works! After probably 3 years and and one point throwing the whole thing in the trash and starting over. Different story for a different day...

Anyway, I am trying to wire Mammoth Electronics' 3PDT footswitch PCB daughterboard and I can't seem to get it working. I've never really been able to get my head around the footswitch wiring that is shown on the GGG layout, and adding this daughterboard creates an additional layer of confusion for me. Does anyone have any experience with this board? Which pads on the switch go to which circuit pads, jacks, etc? There's no schematic on Mammoth's site.

Thanks so much! Mammoth's site is blocked here at work so I can't try it. So now I'm gonna do some soldering when I get home instead of jumping off a bridge along the way. You saved me!

I think manufacturers have different specs for which lugs do what. NONE of the ones on the net were according to the switches I have. My switches are wired just like this:
