What do you want, exactly?
Paia has a "tube mic pre" That runs on low-voltage and uses a hybrid design, using the tube to "warm things up".
There are also a LOT of good tube mic preamp designs out there for sale or DIY (do a search on Gyraf audio). Also check out the Jensen site (transformer manufacturer with good example schems)
Old Altec stuff is simple but okay, too.

The problem is IMHO a tube mic pre only makes sense if you also use *very* good in-and output transformers which are very expensive. See Jensen, for those.
Once you have the transformers, it's basically a low-noise amp, maybe with added phase switching, phantom power and ground lift...
I wouldn't build anything with tubes on a PCB - these things get HOT and pcb material doesn't like that.
